Winooski Avenue and North Street in Burlington's Old North End. The all-volunteer staff saw approximately 50 patients per week.

Pearl Street Youth Health CenterThe Pearl Street Youth Health Center serves teens and young adults aged 13-25. Also located in downtown Burlington, Pearl Street is located directly next to the Spectrum Drop-In Center for homeless and at-risk youth, creating easy access for a population served by both organizations.
School-Based Dental CenterThe Integrated Arts Academy at the H.O. Wheeler School in Burlington's Old North End Houses our School-Based Dental Center
School-Based Dental Center EntranceThe entrance to the School-Based Dental Center, where any income eligible or uninsured student in the Burlington School District can be bussed in to access comprehensive dental care, alleviating transportation barriers for many parents.
Champlain Islands Health CenterFormerly the Keeler Bay Health Center, a brand new building warranted a brand new name. The improved Champlain Islands Health Center allowed us to expand by three exam rooms, and has space for future dental operatories pending funding.
South End Health CenterFormerly Burlington Primary Care, our South End Health Center expands access to patients on the opposite end of town from our Riverside Health Center.
GoodHEALTH Internal MedicineOur first and only Internal Medicine practice, South Burlington's GoodHEALTH joined the CHCB family in 2016.
Winooski Family HealthOur eighth health center location, Winooski Family Health came on board in 2017, expanding access to our services for residents of a Medically-Underserved Area.
