Sebasticook Family Doctors is a Federally Qualified Health Center located in rural central Maine with five locations: Dexter, Dover, Pittsfield, Newport, and Canaan. Established in 2003, we have continued to expand our services to meet the needs of our community and currently serve 9,000 patients. SFD is PCMH NCQA Level 3 recognized and we provide high quality of care. SFD participates in the Pioneer Accountable Care Organization as well as Maine Health Home and Community Care Teams which expand the resources for our organization as well as the people that we serve.
We offer Medical, Dental and Behavioral Health s services to those who have insurance, are underinsured with high deductibles, or who have no insurance. Our programs are designed to reduce patients' barriers to health care, and include: a 340B drug program, a prescription assistance program, sliding fee scale program, case management, outreach, and information about and enrollment in the Health Insurance Marketplace.
Sebasticook Family Doctors does more than deliver health care service to their patients. They also serve as one stop shopping for other needs of the patients by linking them to a wide variety of community services that will improve the patients' overall well-being.
We support our community and are always looking for ways to give back, for example providing dental services in school, holding fundraisers for community programs in need, offering free community spaghetti suppers.
