Sunrise Community Health began as a migrant health center in 1973. Today, Sunrise operates 11 clinics in northern Colorado in Weld and Larimer Counties, including 9 clinics in the Greeley/Evans area and 2 clinics in Loveland. In 2019, Sunrise supported the health of nearly 42,000 patients, 91% of whom live at an income level indicating they face barriers to affordable health care.
Our dedicated and compassionate team of 420 people provide quality, integrated, and affordable medical, dental, optometry, and behavioral health care. Our culturally competent and comprehensive care model includes on-site laboratory, radiology, pharmacy, health education, interpretation, and care management services. Sunrise is also a recognized leader in collaborations, quality care, HIT, patient-centered care management, and educational health center activities. All services are offered to people in need regardless of their ability to pay.
